Phyllis Szotak

SZOTAK, Phyllis, S., 65, of Kettering passed away Wednesday February 8, 2006 at Hospice of Dayton. She is preceded in death by her parents Joseph and Mary Novak. Phyllis is survived by her son Michael (Beverly) Szotak; daughter Mary (Robert) Thieman; grandchildren Joseph M. Thieman, Tony Thieman, and Alexander Szotak; brother Thomas J. (Pat) Novak; many other family and friends. She was a nurse for Hospice in Phoenix, AZ, chemist, and graduate of Julienne Class of 1958. Funeral Mass will be held 10:30AM Monday February 13, 2006 at Church of Ascension, 2025 Woodman Dr., Kettering. Family will receive friends from 5-7PM Sunday February 12, 2006 at Newcomer Funeral Home, 3940 Kettering Blvd. In lieu of flowers contributions may be made to Hospice of the Valley Pediatric Team 1510 E.
