She was born on March 9, 1933 in Faxon, TN to Elvis and Lorene Berry.
She is preceded in death by her husband, Fred Kozak; daughter, Karen Alvey; parents; 2 sisters; and a brother.
Survived by her son, Jim (Pam) Kozak and grandchildren, Keenan and Brittany.
